Members of South Shore Community Partners in Prevention participated in a forum entitled "From Assessment to Action: Conversation About Health In Our Communities" on September 14, 2011, in Marshfield, MA. The event served as the beginning of a process to outline action steps to address the health priorities of CHNA 23. Southeast Center for Healthy Communities Health Analyst Bonnie Andrews presented the results of a community health assessment conducted for the towns of Carver. Duxbury, Halifax, Hanover, Hanson, Kingston, Marshfield, Pembroke, Plympton and Rockland. Jordan Hospital Vice President of External Affairs Andrea Holleran then described their current efforts to address obesity through nutrition and activity approaches. Healthy City Fall River coordinator Dr. David Weed then gave a keynote presentation on the assessment, planning and organizational process of CHNA 25's approach to these issues. Following a discussion of the social determinants of health framework by Community Health Specialist Deepani Jinadasa, the group then divided into breakout groups to begin to discuss access to care issues for special and vulnerable populations.
