Action Plan Participant:  Visiting Nurse Association of Southeastern Massachusetts

The VNA and Fall River Police Dept. Health & Wellness program utilizes new telemedicine technology to monitor vital signs. Provide monthly lectures on various health topics, and provides individualized counseling on healthy lifestyle choices

Goal:

Individual participants set goals on weight loss, blood pressure control, or increased knowledge of health. The VNA of SE MA is committed to the the providion of comprehensive health services in the home setting and to developing community-based programs which foster improved health.

Method(s):

Telemedicine-Vital sign monitoring up to three times a week (BP, pulse, weight, blood O2), lectures on health (hypertension, diabetes, stress, stroke) individualized counseling on risk factors and lifestyle choices. Development of health programs based on community or individual needs utilizing skilled nursing, rehabilitative therapies, or telemedicine technology.

Timeline:          

March 2005 - March 2006 with possible extension

Indicators:        Success will be determined by participants achieving goals R/T weight loss or blood pressure control. Overall goal will be to increase knowledge and awareness of healthy lifestyle choices.

Monthly lectures are open to all Police Dept. personnel, their family members and retirees.
